  • Hospice Medical Director

    Our hospice medical director ensures that the hospice team is supported and that the care goals set for each patient are achieved, ensuring that every one of the patient’s care wishes is followed.

  • Chaplain Services

    When a patient is dealing with a critical illness or is nearing the end of life, our hospice chaplain can address the patient’s spiritual needs.

  • Skilled Nursing

    Our skilled nurses are highly-trained in pain and symptom management. They are available around the clock for any change of condition.

  • Assistance with Daily Living Activities

    We offer assistance with activities of daily living (ADLs) during Hospice Aides visits.

  • Counseling and Community Services

    Resource referrals provided by our Social Workers.

  • Physical, Occupational and Speech Therapy Services

    We offer physical, occupational and speech therapy services as needed.

  • Nutrition Counseling

    Our dietitians can offer nutrition counseling, ensuring that the patient’s nutritional needs are addressed. They can perform a nutritional evaluation of the patient, offer recommendations for nutrient intake, and educate patients and family regarding hydration/nutrition issues.

  • General Inpatient Care

    We provide general inpatient care while in a hospice skilled nursing facility as needed for pain control and symptom management, as well as respite for family or caregivers.

  • In-Home Continuous Care

    We provide in-home continuous care as necessary for crisis situations.

  • Bereavement Counseling

    Hospice doesn’t end once your loved one passes away. We offer bereavement counseling for family members who are finding it difficult to cope after their loss.

What is Hospice?
Hospice is a health care system of interdisciplinary services provided to patients and their families during the final stages of life. It is a benefit program designed to assist with the special needs of patients and their loved ones when the patient is diagnosed with a condition for which they elect comfort and palliative care.
How is Hospice Care provided?
Hospice care is most often provided in your home, where the environment is familiar and loved ones are near. It can also be provided in nursing homes and other care facilities. Hospice professionals provide medical and nursing care, emotional, and spiritual support to the patients and family. Hospice also assists patients’ family/loved ones during the grieving process.
Is Palliative Care the same as Hospice Care?
Yes, the principles are the same. Palliative care is the care of patients with active, progressive, advanced disease or a condition with a short life expectancy for whom the focus of care is the relief and prevention of suffering that then results in an enhanced quality of life.
Does Hospice Care Make a Difference?
Yes, hospice care can dramatically and positively impact the quality of life. The goal is to provide relief from pain and physical symptoms and to support the family during bereavement.
How is a Patient Admitted to Hospice Care?
Any person concerned with a patient’s well-being can submit a referral. Just call Assure Hospice Care and a staff member will arrange to meet the patient/family for an explanation of hospice benefits. Once eligibility is determined, all hospice services requested will be provided immediately.
How is Eligibility for Hospice Care Determined?
